Accessing Your Health Care Records
Clients have the right to request copies of their health care records in accordance with applicable state and federal law.
To request records, please contact our Practice Owner and Clinical Director , Chelsea Fielder-Jenks, directly or submit a request through our Contact page. Once we receive your request, we will provide guidance on the appropriate next steps and required documentation to ensure your privacy and the security of your records.
Requests are processed in accordance with HIPAA and Texas law. Verification of identity may be required prior to release of records.
